Dental Implants

Although implant dentistry was first introduced as a tooth replacement option for people that didn’t have any teeth, dentists immediately saw the chance to use this method to replace a single missing tooth. The most common missing tooth is the lower first molar.

Previously, the only treatment option for patients with a single missing tooth was to create a bridge, which involves grinding down the adjacent teeth. This is still a common practice. The problem is that the teeth being ground down are usually perfectly healthy, but are destroyed in order to replace one missing tooth. Also, a bridge only replaces the visible part of the tooth, but not the root. This caused problems during chewing, as the pressure from the missing tooth is re-directed to the adjacent teeth, putting stress on those teeth.
A dental implant, on the other hand, involves the use of an artificial root, which supports the visible part of the tooth replacement without interfering with the healthy teeth on either side. Dentists now feel that replacing a missing tooth with a dental implant is actually a healthier and more conservative treatment than traditional bridges.

In the case shown here, the adjacent teeth are healthy and have no reason to be crowned, so dental implants have been used to replace the missing tooth. The anchor of the implant is visible on the x-ray.

Here, the implant abutments, or artificial roots, can be seen fixed into position. The final step is to cement the crowns into place.

In this photo, the patient has lost their lateral incisor. A removable bridge has been created to fill the space as a temporary measure, but it is not a reasonable long-term solution, nor is a permanent bridge. In this case, the two front teeth already have porcelain crowns on them, so in order to use a resin bonded bridge, one of the crowns would need to be removed and the healthy canine tooth behind the space would need to be ground down. Even if a bridge supported by the adjacent teeth was chosen, it would not have worked predictably and there would have been difficulty matching the shade of the crown with the other front tooth.

This x-ray shows the implant abutment in place, protruding through the gum tissue so that a crown can be affixed to the end. In the photo, the finished result can be seen. The right lateral incisor is the implant, but clearly one would not be able to tell by looking at it.

This patient lost six lower front teeth in a car accident. At the time of the accident, a removable bridge was the only treatment choice available. Due to the number of teeth missing in this case, a fixed bridge will not work, even with grinding. There is simply nothing to support it with. With the patient’s lip pulled back, the partial denture that holds the false teeth is visible, along with part of the empty gum below. Ten years after the accident, dental implants allowed this patient to have the missing teeth effectively replaced.
